A one-time enrollment fee, equal to the monthly membership fee, is waived if you choose to pay for a yearlong membership in advance. The enrollment fee and first month's membership are due at the time of your first visit. All memberships are subject to approval. A pre-enrollment meeting with Dr. Diamond is required for all prospective patients.
